Run a specific shift briefing, plan the work and follow a hazard through to closure. This editable Word pack gives host supervisors three connected forms for agency workers in warehousing, manufacturing and logistics: a toolbox talk run sheet, a job safety analysis (JSA) and a hazard or near miss report.

Make the briefing useful on the floor

A toolbox talk should invite the people doing the job to point out changes and gaps. Use the facilitator prompts at the work area, then carry unresolved items into the action register. The JSA names a control owner and evidence for each task step. The report form tracks interim protection while longer-term corrective actions are checked.

WorkSafe Victoria describes consultation, cooperation and coordination between hosts and labour hire providers where they share OHS duties to the same workers. SafeWork NSW advises hosts to explain hazard and incident reporting and to consult workers and the labour hire PCBU before changing work scope or location. See the primary guidance below.

Frequently Asked Questions

Can I use the same talk for every shift?

Reuse the format, not yesterday’s assumptions. Confirm the people, work area, task and changed hazards with the crew each time.

Who should complete the JSA?

The host supervisor should work with the workers doing the task and record the agreed controls and pre-start approval. Involve the agency contact when scope or controls change.

Is a near miss worth recording if no one was hurt?

Yes. Use the report to capture what happened, immediate protection, follow-up actions and feedback to the worker who raised it.

Does this pack make our site compliant?

No. It is an editable record set, not a substitute for consultation, implemented controls, site procedures or advice on the rules that apply to your work.
